Pre-K Round-Up Enrollment CheckList:
1. Enrollment Form Completed
2. Copy of All Parent/Guardian Drivers License
3. New Student Enrollment Must Have A Copy of:

● Copy of Birth Certificate
● Copy of Immunization Forms (current) or
Exemption Form
● Copy of Electric Bill with Name & Address
(No water bills) or a Rental Agreement
● Copy of CDIB Card (If applicable)
